Hamiltime Herb Co. LLC is facing $152,829 in fines for safety and health hazards, said officials at the Occupational Safety and Health Administration (OSHA).
The investigation was a follow-up investigation from incidents in July and August last year at the Howell, New Jersey, facility.
The citations include five for failure to abate and two serious violations.
Investigators cited the pet food manufacturer for failing to develop a lockout/tagout program to prevent unexpected machine startup, and a respiratory protection program for employees required to wear tight-fitting respirators; exposing employees to unguarded machinery; and failing to adequately train and certify employees to operate a forklift.
The inspection was a follow-up to two investigations last year when an employee had four fingers amputated when a batch mixer activated while being cleaned.
