A worker who died after a heavy steel plate fell on him in an industrial accident at the Caterpillar facility in Milwaukee has been officially identified, officials said.

Steven Wade, 60, of Oak Creek, WI, died from his injuries Monday, four days after the May 10 accident, said the Milwaukee County Medical Examiner.

Wade was welding plates when police received a call at 2 p.m. saying Wade had been seriously injured in an accident, officials said. 

The South Milwaukee Police Department, Milwaukee County District Attorney’s Office and Occupational Safety and Health Administration are all investigating the incident.

Caterpillar issued a statement following the tragic accident:

“We are deeply saddened by the death of our employee who was involved in an accident in our South Milwaukee facility on May 10. Our thoughts and prayers are with this employee’s family, friends and co-workers. The company has arranged for counseling to be available to employees who may request it.”
